Purpose

Sneh Children Hospital started the hospital with an intent to offer the best health to your child, with the most effective medical knowledge and infrastructure possible. We cater to all our pediatricians and young sufferers throughout Gujarat. The hospital was established with 3 of the best and senior pediatric specialists. We’ve inbuilt “state-of-the-art” medical infrastructure and facilities to make sure that each little one who’s admitted beneath our care goes back pleased, healthy, and sound. We have  15 bedded intensive care unit, with level III NICU services and 10 bedded intensive care unit, with level III PICU services.

Sneh Children’s Hospital additionally recognizes it’s a bigger responsibility to create social and medical consciousness which may go a great distance in preventive well being care. Personal details (name, address, date of birth, email address, phone numbers, photos); Your child’s medical history; Notes made during the course of a medical consultation; Referral to other health services providers; Results and reports received from other health service providers; and a Credit card or direct debit information for billing purposes. We only collect this information when you transect this with us either at the practice over the phone call, via e-mail, or via written correspondence. In some instances, if needed, we collect information from other parties wherever you have been a patient, either it is referring doctors, treating specialist, pathology, radiology, or other health care providers. In an emergency, we collect information from immediate family, friends, or relatives.

To the professional team directly involved in your health care, including treating doctors, pathology services, radiology services, and other specialists outside this practice. This may occur through referrals when requesting for certain tests or in the report or result returned to us following the referrals. To your health insurance fund, Medicare or other organizations responsible for the financial aspects of your care; To insurers or lawyers for the defense of a medical claim; and/or To assist with the training and education of other health care professionals. Information quality/Amendment of your Personal Information:

We aim to ensure that the information we hold about is accurate, relevant, complete, and up-to-date. If you are changing any of the information as per your convenience it is required to update us in advance during the consultation or over the phone call. Our medical staff confirms all the details provided by you regarding your contact detail, place of address, identity proof, etc.

Access to your Personal Information:

You have all the right to access all the information displayed on our Sneh Children Hospital website, either it is written, electronic, including video & photograph. To access your records, you must inform us in writing of your request. There are no charges for providing you with copies of your information. Storage:

Our medical practice takes all the steps to protect the security and personal information we hold; by: Securing our premises; Using passwords on all electronic systems and databases and varying access levels to protect electronic information from unauthorized interference, access, modification, or disclosure; and Storing hard copy records in secure filing cabinets or rooms that are accessible only to Practice staff.

Comments:
